What is Mednovate Connect?
Using the Illuminate.Health digital assistant, patients work with their pharmacist to create an accurate medication and dynamic care plan to avoid drug-related problems.
Your personal clinical pharmacist will establish a medication monitoring plan, which includes goals you can track in real-time. Your medication record, accessible to you through the Illuminate.Health app, is made available to all of your physicians. You can use the app to schedule virtual appointments with your pharmacist, ask drug-related questions, and monitor your progress.
